C3 Collaborative

The C3 Collaborative is a Clinical-Community Collaborative consisting of various members from the public health, community, and health care setting.

Mission Statement:

“Connecting Clinics with Communities.”

This collaborative was established with a goal of improving health outcomes for children and decreasing the rates of childhood obesity in two counties in WV—Fayette and Kanawha County.

Strives to create Clinical-Community linkages, which integrate clinical and community systems to provide better access and resources to individuals and families. The goal is to connect healthcare providers with the community.

Meets monthly via zoom from 12:30-1:30 on the 3rd Thursday of every month

Current C3 Initiatives

Project ECHO for Pediatric Obesity

Monthly live Zoom webinars for providers/healthcare professionals regarding obesity/public health topics.
The webinars are recorded and converted to a podcast for those not able to attend the live webinar.
Physicians, nurses, dietitians, social workers, psychologists and pharmacists who attend the live webinar or listen to the recorded zoom and complete a Post Test receive CME credit.
The webinars are held on the 4th Wednesday of every month from 12:00-1:00 pm.
This initiative started in January 2022.

5210 Marketing Campaign

KEYS hired JJN Multimedia to develop a 5210 marketing campaign to disseminate the healthy message of 5210.
As part of this campaign, JJN created a new 5210 logo; an original 5210 YouTube song/dance video; rented two billboards for advertising 5210; created a specific section with 5210 information on KEYS 4 HealthyKids’ new website; and developed materials, including a poster and USB with education about 5210, to distribute to schools.
This campaign launched in January 2022.
