Diabetes Self-Management Program (DSMP)

Mutual support and success builds participants’ confidence in their ability to manage their health and maintain active and fulfilling lives.  Participants make weekly action plans, share experiences, and help each other solve problems that they encounter in creating and carrying out in their daily lives.   

Program Goals

    • hyper/hypoglycemia

    • fear

    • frustration 

  • Exercise to maintain and improve strength 

  • Healthy eating 

  • Appropriate use of medication 

  • Working more effectively with health care providers 

  • Share experiences and help each other solve problems
