National Diabetes Prevention Program (NDPP)

Program Goals 

  • Eat healthy without giving up all the foods you love

  • Add physical activity, even if you don’t have time

  • Cope with challenges that can derail your hard work

    • like how to choose healthy food when eating out

  • Get back on track if you stray from your plan

    • because everyone slips now and then!


  • Age of 18+ 

  • BMI of 24+ 

  • A positive lab result within the last year: 

    • HbA1C: 5.7-6.4% OR 

    • FPG: 100-125 mg/dL OR 

    • OGTT: 140-199 mg/dL OR 

  • High risk for prediabetes (CDC or AMA screening test)


  • History of gestational diabetes  

Empowers patients with prediabetes to take charge of their health and well-being through incorporating healthier eating and moderate physical activity, problem-solving, stress-reduction and coping skills into participants' lives.
