Eligibility
-
Age of 18+
-
BMI of 24+
-
A positive lab result within the last year:
-
HbA1C: 5.7-6.4% OR
-
FPG: 100-125 mg/dL OR
-
OGTT: 140-199 mg/dL OR
-
-
High risk for prediabetes (CDC or AMA screening test)
OR
-
History of gestational diabetes

Empowers patients with prediabetes to take charge of their health and well-being through incorporating healthier eating and moderate physical activity, problem-solving, stress-reduction and coping skills into participants' lives.
