Funding opportunity for West Virginia local health departments to help enhance clinic and community

We are looking for local health departments invested in effectively addressing priority health issues (prediabetes, diabetes and cardiovascular conditions) through collaboration, innovation, and a shared vision for a healthier state. These resources are being made available thanks to a grant from the Centers for Disease Control and Prevention to the West Virginia Bureau for Public Health - Division of Health Promotion and Chronic Disease (HPCD). HPCD is investing in West Virginia local health departments and, in effect, investing in the future of our state. This grant is designed to help our state successfully address chronic disease prevention and control for these priority health conditions.


As a recipient of these grant funds, your organization is expected to enroll and participate in WV Health Connection. WV Health Connection is a collaborative effort led by the West Virginia Bureau for Public Health, Division of Health Promotion and Chronic Disease and West Virginia University, School of Public Health, Office of Health Services Research. WV Health Connection is a system for patients to discover community wellness programs that are convenient and accessible. To do this, WV Health Connection links healthcare providers to local chronic disease prevention services and connects patients to health-promotion programs.


We will be accepting submissions starting Monday, November 18, 2019. Nine grantees (local health departments) will be awarded $10,000 in 2020 to implement this work. The deadline for this application has been extended to Monday, December 16, 2019 by 5:00 PM. We will be reviewing these applications soon after the submission and your organization will receive a decision notification by Monday, December 23, 2019.
