The Synergy 2.0 Partners shown below formed a partnership to address chronic disease among West Virginians.  More specifically, the Synergy Partners team works together to expand and strengthen diabetes and cardiovascular disease prevention and management programs across the state.  The WV Health Connection initiative is one piece of this greater initiative.
